The first step in any journey tends to be the hallmark of change and we are looking forward to being part of this process with you.
When you are ready to begin please contact us via phone or email in order to schedule an initial appointment and a representative will assist in finding a time that is convenient to both parties availability.
During the initial session you will receive some preliminary forms to complete which include demographic information, insurance, policies and general questionnaire. We request that you arrive 10 minutes early to your first appointment in order to facilitate this process.
Insurance
We are in-network with Blue Cross/ Blue Shield PPO and Blue Choice.
For out-of-network insurance plans, we can provide you with a monthly billing statement upon request. We suggest contacting your insurance company if you should have any questions regarding your company's out-of-network benefits.
Payment
We accept cash, check and all major credit cards as forms of payment.
Cancellation Policy
If you are unable to attend a session, please contact us as soon as possible.
You may be charged a late cancellation fee for sessions cancelled less than 24 hours from the scheduled session time.
